Accreditation

  • To receive CME credit, please complete the evaluation form by scanning the QR code.
  • The evaluation must be submitted no later than Friday, June 5th at 5pm.
  • Be sure to enter the correct email address on the form, as your CME certificate will be sent to that address by our CME office.
  • Please allow up to two weeks for processing and distribution of certificates.
